1. Purpose of This Form

This Client Consent & Liability Waiver (“Agreement”) outlines your understanding and acceptance of the services provided by Aamir Wellness & Research Center (hereafter referred to as “Aamir Wellness”, “we”, or “our”) and your responsibilities as a client or participant.

By signing or submitting this form, you confirm that you voluntarily choose to participate in a consultation, wellness session, or herbal guidance program offered by Aamir Wellness.

2. Nature of Service

I understand that:

  • Aamir Wellness provides herbal, nutritional, and lifestyle consultations for the purpose of improving general wellness, vitality, and balance.
  • The services provided are educational and supportive in nature, based on traditional Unani, herbal, and naturopathic principles combined with modern phytomedicine knowledge.
  • These services are not a substitute for licensed medical care, diagnosis, or treatment.

I acknowledge that practitioners at Aamir Wellness are not medical doctors, and they do not prescribe or adjust allopathic medications.

3. My Responsibilities as a Client

I agree to:

  • Provide complete and accurate information about my health, medications, and lifestyle.
  • Inform my practitioner of any changes in my condition or symptoms.
  • Take personal responsibility for how I use the recommendations provided.
  • Seek immediate medical attention in case of emergency or severe health issues.
  • Understand that my participation in this program is voluntary and I can withdraw at any time.

4. Consultation & Product Disclaimer

I understand that:

  • Any herbal formulas, supplements, or lifestyle plans recommended to me are based on my individual needs and assessment.
  • Results may vary from person to person depending on factors such as age, constitution, and adherence to the plan.
  • I am responsible for informing the practitioner of any allergies, medical conditions, or adverse reactions I may experience.
  • Aamir Wellness is not liable for any side effects, allergic responses, or unexpected outcomes arising from my use or misuse of herbal or wellness recommendations.

5. Confidentiality & Data Privacy

I acknowledge that:

  • All personal and health-related information I provide will be treated as strictly confidential in accordance with Aamir Wellness’ Privacy Policy.
  • My information will only be used to deliver the agreed-upon services and will not be shared with third parties except as required by law.

6. Online Consultation Terms

If my session is conducted online (via Zoom, WhatsApp, Google Meet, or other platforms):

  • I agree to participate in a private, distraction-free environment.
  • I understand that technical issues (internet disruptions, connectivity problems, etc.) may occur, and Aamir Wellness will offer to reschedule if necessary.
  • I agree not to record, publish, or share any part of the consultation without prior written consent.

7. Fees, Payments, and Refunds

  • I understand that consultation fees must be paid in advance and are non-refundable once the session has been completed.
  • Refunds may only be issued in cases of verified technical failure, double payment, or practitioner unavailability, as stated in the official Refund & Return Policy.
  • I agree to abide by Aamir Wellness’ published rescheduling and cancellation terms.

8. Acknowledgment of Risk

I acknowledge that:

  • All herbal and wellness guidance involves individual variability and inherent risk.
  • Aamir Wellness makes no claims of guaranteed results or outcomes.
  • By participating, I accept full responsibility for my own health decisions and any risks associated with using herbal, dietary, or natural therapies.

9. Release of Liability

I hereby release and discharge Aamir Wellness & Research Center, its founder(s), practitioners, employees, and affiliates from:

  • Any and all claims, damages, or liabilities resulting from my participation in consultation or use of any product or guidance provided.
  • Any loss or injury caused by misunderstanding, misapplication, or adverse reaction to any herbal or lifestyle suggestion.

I understand this release applies to any known or unknown claims at the time of signing.

10. Consent to Communication

I consent to receive:

  • Follow-up messages, health updates, or wellness recommendations via email, WhatsApp, or SMS.
  • Occasional educational newsletters or promotional materials (which I can unsubscribe from at any time).

11. Governing Law

This Agreement is governed by the laws of Pakistan, and any disputes shall be resolved in the courts of Bahawalpur, Punjab.

12. Declaration of Understanding

By signing or submitting this form, I declare that:

  • I have read and fully understood the contents of this document.
  • All my questions have been answered to my satisfaction.
  • I have voluntarily agreed to receive wellness or herbal consultation services from Aamir Wellness.
  • I understand that I am free to discontinue services at any time.
